<p><strong>INFORMATION ABOUT THE QUESTIONNAIRE SURVEY (INFORMATION SHEET)</strong></p> <p><strong>About the project</strong></p> <p>This survey is part of a research project funded by the European Union and carried out by Charles University (Prague, Czechia) and Menon Economics (Oslo, Norway) in collaboration with several other research institutions. <p>We will ask you about a range of topics related to your health and perception of chronic illnesses. The survey does not intend to recommend any commercial product, treatment, or service. The project results aim to help streamline policies that improve public health. <p><strong>Length of questionnaire completion:</strong></p> <p>We estimate that completing the questionnaire will take you approximately 20 minutes. <p><strong>Who is responsible for this study and data collection? </strong></p> <p>This research is led by Professor Ståle Navrud from Menon Economics, e-mail: stale.navrud@menon.no. <p>Recruitment of survey participants is performed by National Sample Ltd. in collaboration with Bilendi, Norstat, Main Frame, Quest Mindshare, Wolf Insight and Intra Research (henceforth “online panel operators”). <p><strong>What are your rights as a participant in the questionnaire survey? </strong></p> <p>Participation in this questionnaire survey is entirely voluntary. If you do not wish to answer certain questions, you can choose the option "I do not want to answer." You have the right to end the survey at any time by closing the browser window, and you may or may not return to the survey. <p><strong>What are the risks associated with participating in this questionnaire survey? </strong></p> <p>There is a minimal risk associated with participating in this questionnaire survey, comparable to browsing the internet. Your personal data as a member of the online panel (including name, address, phone number, and email), which is held by your online panel operator, will not be part of any activity carried out within this study and will not be provided to the research team members under any circumstances. Your online panel operator will randomly assign you an identification number at the beginning of the questionnaire. Researchers who will check and analyse the results of the questionnaire survey and the answers you provide will only see this random identification number. Your online panel operator will not provide them with any information that would allow them to link this identification number to you. <p>All reports or professional publications based on this questionnaire survey will contain only aggregated statistical information, such as averages, variances, etc. No specific answers that could be retrospectively linked (or profiled) to a specific person will be published under any circumstances. Data collected in the survey may be provided to third parties, but only to verify the study results. <p><strong>What are the benefits of participating in this questionnaire survey? </strong></p> <p>As a member of the online panel, you will receive rewards for completing this survey. Apart from these rewards, participation in this study has no other personal benefits for you. However, the research project, of which this questionnaire survey is a part, may help in designing public programs and policies that focus on public health and a low-pollution environment. Forskere, der vil kontrollere og analysere resultaterne af spørgeskemaundersøgelsen og de svar, De afgiver, vil kun se dette tilfældige identifikationsnummer. Deres onlinepaneloperatør vil ikke give dem oplysninger, der ville gøre det muligt at knytte dette identifikationsnummer til Dem. <p>Alle rapporter eller professionelle publikationer baseret på denne spørgeskemaundersøgelse vil kun indeholde aggregerede statistiske oplysninger, såsom gennemsnit, variationer mv.
