<p><strong>INFORMATION ABOUT THE QUESTIONNAIRE SURVEY (INFORMATION SHEET)</strong></p> <p><strong>About the project</strong></p> <p>This survey is part of a research project funded by the European Union and carried out by Charles University (Prague, Czechia) and Menon Economics (Oslo, Norway) in collaboration with several other research institutions. <p>We will ask you about a range of topics related to your health and perception of chronic illnesses. The survey does not intend to recommend any commercial product, treatment, or service. The project results aim to help streamline policies that improve public health. <p><strong>Length of questionnaire completion:</strong></p> <p>We estimate that completing the questionnaire will take you approximately 20 minutes. <p><strong>Who is responsible for this study and data collection? </strong></p> <p>This research is led by Professor Ståle Navrud from Menon Economics, e-mail: stale.navrud@menon.no. <p>Recruitment of survey participants is performed by National Sample Ltd. in collaboration with Bilendi, Norstat, Main Frame, Quest Mindshare, Wolf Insight and Intra Research (henceforth “online panel operators”). <p><strong>What are your rights as a participant in the questionnaire survey? </strong></p> <p>Participation in this questionnaire survey is entirely voluntary. If you do not wish to answer certain questions, you can choose the option "I do not want to answer." You have the right to end the survey at any time by closing the browser window, and you may or may not return to the survey. <p><strong>What are the risks associated with participating in this questionnaire survey? </strong></p> <p>There is a minimal risk associated with participating in this questionnaire survey, comparable to browsing the internet. Your personal data as a member of the online panel (including name, address, phone number, and email), which is held by your online panel operator, will not be part of any activity carried out within this study and will not be provided to the research team members under any circumstances. Your online panel operator will randomly assign you an identification number at the beginning of the questionnaire. Researchers who will check and analyse the results of the questionnaire survey and the answers you provide will only see this random identification number. Your online panel operator will not provide them with any information that would allow them to link this identification number to you. <p>All reports or professional publications based on this questionnaire survey will contain only aggregated statistical information, such as averages, variances, etc. No specific answers that could be retrospectively linked (or profiled) to a specific person will be published under any circumstances. Data collected in the survey may be provided to third parties, but only to verify the study results. <p><strong>What are the benefits of participating in this questionnaire survey? </strong></p> <p>As a member of the online panel, you will receive rewards for completing this survey. Apart from these rewards, participation in this study has no other personal benefits for you. However, the research project, of which this questionnaire survey is a part, may help in designing public programs and policies that focus on public health and a low-pollution environment. <p><strong>INFORMACIJE O ANKETNOM ISTRAŽIVANJU (INFORMATIVNI LIST)</strong></p> <p><strong>O projektu</strong></p> <p>Ova anketa je deo istraživačkog projekta koji finansira Evropska unija, a sprovode ga Karlov univerzitet (Prag, Češka) i Menon Economics (Oslo, Norveška) u saradnji s nekoliko drugih istraživačkih institucija. <p>Pitaćemo vas o nizu tema u vezi s vašim zdravljem i percepcijom hroničnih bolesti. Anketa ne namerava preporučivati nijedan komercijalni proizvod, tretman ili uslugu. Rezultati projekta imaju za cilj da pomognu u unapređivanju politika koje poboljšavaju javno zdravlje. <p><strong>Trajanje popunjavanja upitnika:</strong></p> <p>Procenjujemo da će vam popunjavanje upitnika oduzeti otprilike 20 minuta. <p><strong>Ko je odgovoran za ovu studiju i prikupljanje podataka? </strong></p> <p>Ovim istraživanjem rukovodi profesor Ståle Navrud iz Menon Economics-a, e-pošta: stale.navrud@menon.no. <p>Regrutovanje učesnika ankete vrši National Sample Ltd. u saradnji s kompanijama Bilendi, Norstat, Main Frame, Quest Mindshare, Wolf Insight i Intra Research (u daljem tekstu "operateri onlajn panela"). <p><strong>Koja su vaša prava kao učesnika anketnog istraživanja? </strong></p> <p>Učešće u ovom anketnom istraživanju je u potpunosti dobrovoljno. Ako ne želite da odgovorite na određena pitanja, možete izabrati opciju "Ne želim da odgovorim." Imate pravo da prekinete anketu u bilo kom trenutku zatvaranjem prozora pretraživača i možete se, ali i ne morate, vratiti na anketu. <p><strong>Koji su rizici povezani s učešćem u ovom anketnom istraživanju? </strong></p> <p>Učešće u ovom anketnom istraživanju nosi minimalne rizike, uporedive s pregledanjem interneta. Vaši lični podaci kao člana onlajn panela (uključujući ime, adresu, broj telefona i e-poštu), koji su u posedu vašeg operatera onlajn panela, neće biti deo nijedne aktivnosti sprovedene u okviru ove studije i neće biti dostavljeni članovima istraživačkog tima ni pod kojim okolnostima. Vaš operater onlajn panela nasumično će vam dodeliti identifikacioni broj na početku upitnika. Istraživači koji će proveravati i analizirati rezultate anketnog istraživanja i odgovore koje ste dali videće samo ovaj nasumični identifikacioni broj. Vaš operater onlajn panela neće im pružiti nikakve informacije koje bi im omogućile da povežu ovaj identifikacioni broj sa vama. <p>Svi izveštaji ili stručne publikacije zasnovane na ovom anketnom istraživanju sadržaće samo zbirne statističke informacije, kao što su proseci, varijanse itd. Nijedan specifičan odgovor koji bi se mogao retrospektivno povezati (ili profilisati) s određenom osobom neće biti objavljen ni pod kojim okolnostima. Podaci prikupljeni u anketi mogu biti dostavljeni trećim licima, ali samo radi provere rezultata studije. <p><strong>Koje su koristi od učešća u ovom anketnom istraživanju? </strong></p> <p>Kao član onlajn panela, dobićete nagrade za popunjavanje ove ankete. Osim ovih nagrada, učešće u ovoj studiji nema drugih ličnih koristi za vas. Međutim, istraživački projekat, čiji je deo ovo anketno istraživanje, može pomoći u osmišljavanju javnih programa i politika usmerenih na javno zdravlje i okolinu s niskim zagađenjem.
